Discharge Administrator Full Time

The Transfer of Care Hub is a multi-organisational Health and Social care team based within Warrington Hospital with a shared vision to ‘support the residents of Warrington and Halton to live well, independently at home for as long as they are able’. Our primary role is to ensure timely and safe assessment and discharges from Hospital to various primary care/ community settings. The post holder will be expected to work on a rotational basis, the Integrated Discharge Team provides a 7-day service between the hours of 8am – 6pm each day.

The post holder will be responsible for providing effective and efficient clerical and administrative support to an integrated team of health and social care professionals.

The role will involve the coordination and implementation of office procedures and will require a high degree of organisational skills and excellent working knowledge administrative systems. The post holder will often be the first point of contact and a focal point for the services requiring clear communication and liaison skills. They will be responsible for managing and prioritizing their own workload and will be required to use their own initiative.

The post holder will assist with cover, in the absence of other colleagues across the full range of required duties.

Our primary role is to ensure timely and safe assessment and discharges from Hospital to various community settings. The Integrated
Discharge Team provides a 7-day service between the hours of 8am – 6pm each day. The post holder will be expected to work on a rotational basis.
All people who no longer meet the criteria to reside for inpatient care in acute hospitals should be discharged home or to a non-acute setting. The post holder will be assigned to patients on pathways 1, 2 and 3 (those requiring health or social care (or both) and support following discharge) to aid and monitor their discharge and recovery.
