The post holder will carry out triage assessments over the phone for patients across the lifespan, in an efficient and effective manner, implementing the most appropriate intervention to meet the patient’s needs by using best clinical practice and adhering to Cardiff and Vale policies, procedures and guidelines.

- Work in accordance with Nursing and Midwifery Council/ Code of Professional Conduct, Standards for Conduct, Performance and Ethics.
- Possess strong clinical skills and sound judgement appropriate to Out of Hours Patients demands which will be underpinned by experience and theoretical knowledge aimed at assessing the unwell child/adolescent.
- Exercise a high degree of professional autonomy, critical judgement and refer to other members of the multidisciplinary team as appropriate.
- To undertake telephone nursing assessment, providing advice and, if necessary, onward referral to the appropriate agency/professional, for people with a wide range of conditions accessing the Out of Hours service.
- To critically analyse complex information alongside professional skills, knowledge and experience.
- To negotiate with parents (adolescents) the most appropriate course of action to meet their immediate health needs.
