NHS Sheffield Clinical Commissioning Group is a membership organisation comprising 79 GP practices and expects to have a commissioning budget (including primary care co-commissioning funds) of circa £900m making it one of the largest CCGs in the country. An exciting opportunity has arisen to join the Continuing Healthcare team. We require one Learning Disability Registered Nurseto work in the Continuing Health Team at Sheffield CCG.

The Sheffield Continuing Health Care team are an innovative and friendly team who deliver a high qualityservice to the population of Sheffield in order to assess and consider continuing healthcare eligibility. This is a great opportunity to be involved in a team, who are working with the Local Authority and Sheffield Teaching Hospitals, towards a more integrated approach in how we assess and meet an individual?s care needs.

We are looking for an experienced Learning Disability Registered Nurse to work within the CHC team as Nurse Coordinator & Care Manager. Our Nurses look at the whole process, by first determining eligibility for NHS funding through utilisation of the Decision Support Tool. The work is at times challenging but also rewarding. The position available is full time permanent and will be a valuable professional development opportunity for applicants wishing to gain experience in Continuing Health Care and care management of individuals with complex ongoing care needs to inform their future practice.
